Unity Hospice and Palliative Care believes that end-of-life care should be comfortable, meaningful, and deeply supportive. Hospice does not mean that life stops—it means focusing on comfort, peace, and quality of life, even while living with a serious illness. Unity works closely with patients and families to create personalized care plans designed to support each individual’s physical, emotional, and spiritual needs. Care is delivered wherever the patient resides, most often at home, and is typically provided at little to no cost.
Our interdisciplinary care team is guided by Unity’s Medical Director and includes registered nurses, certified nursing assistants, social workers, chaplains, and trained volunteers. Patients receive expert pain and symptom management, specialized medications, and medical supplies delivered directly to their home. Durable medical equipment such as hospital beds and oxygen is also provided and set up by experienced professionals. In addition to scheduled visits, Unity’s care team is available 24/7 to respond to changing needs and provide reassurance at any time.
Support at Unity extends beyond medical care to encompass the entire family. Social workers and nurses offer education, emotional guidance, and assistance with caregiving and end-of-life planning. Respite care options provide temporary relief for caregivers, while ongoing collaboration ensures families feel supported throughout the journey. After a loved one’s passing, Unity’s bereavement team continues to provide grief support, counseling, and memorial services for up to twelve months, ensuring families are never alone during the healing process.
